Transition Application Procedure (Associate Degree in Nursing)

You must have your application completed and turned in by October 1st of each calendar year to be considered for the following Spring Semester.

It is your responsibility to provide all of the documents requested. Due to privacy and legal policies along with the number of applicants, we will not contact any office on campus requesting information for you (This includes Eastern transcripts and ACT scores).

It is also your responsibility to call and verify that the Nursing Department has received all the required forms to complete your application. There will be no type of mailing or a call to request the information from you.

The following documents must be returned to complete you application.
  • Proof of your LPN licensure
  • A copy of your high school transcript or GED
  • A copy of transcripts from all of the colleges you have attended
  • A copy of your ACT scores
  • GPA

If you graduated from LPN school before 1994, it is imperative that you take the advanced standing exams prior to the January enrollment. The test categories are:

Fundamentals I & II, Nursing of the Childbearing Family, and the Nursing of Children.

You have two opportunities to pass these exams. Retesting can be done 60 days after the initial testing date. Please check with the Nursing Department for test dates.
