Employee Grievance Form

All complaints or any concerns about conduct that may violate Eastern Oklahoma State College's harassment or sexual harassment policy, as well as any form of retaliation should be filed with the Title IX Coordinator using the form below.

You may be assured that you will not be penalized in any way for reporting a harassment problem. All complaints of harassment that are reported will be investigated as promptly as possible and corrective action will be taken where warranted. Eastern Oklahoma State College prohibits employees from hindering an internal investigation and complaint procedure. All complaints of harassment that are reported will be treated with an appropriate level of confidentiality. The need for confidentiality will be determined on a case-by-case basis.

Your notification of the problem is essential. A harassment issue cannot be addressed if the proper individuals are not informed about it. Therefore, it is your responsibility to bring those kinds of problems to the institution's attention so that the necessary and proper steps can be taken to correct the problem.

Complaints may be submitted anonymously; however, unless you include your contact information Eastern Oklahoma State College may be unable to investigate your complaint and will not have the ability to respond back to you regarding an incident.

Types of Complaints

Please provide detailed information and choose one of the following options based on the type of incident you are reporting:

General Incident – For general concerns or actions that are experienced or observed involving a student, faculty or staff member.

Safety Concern or Threatening Behavior – For concerns or actions that are experienced or observed and present a danger to an individual or the EOSC community. If this is an emergency, please contact Campus Police immediately at 918.465.1718.

Description of Incident or Concern

Please provide a detailed description of the incident/concern using specific concise, objective language (who, what, where, when, why, and how). Please also include any history of the incident (what steps have been taken previously to remedy this situation? What has been contacted and when?)
