Eastern Oklahoma State College honors two Distinguished Alumni each year at the Sapphire Ball. The annual black-tie event raises funds for student scholarships while honoring the college's alumni, faculty, staff and community supporters.

Nominations are based on the following criteria. Please provide as much detailed information and supporting documents as possible. Supporting documents such as resume, letters of recommendation, newspaper articles, excerpts from professional journals, etc. may be attached to this nomination form.

1. Professional Achievements: The nominee's employment and career history. Includes honors and awards received that are related to the nominee's career.

2. Service to the Community: Volunteer services performed by the nominee. Includes awards and recognitions received by the nominee for volunteerism and service.

3. Service to EOSC: The nominee's contributions to EOSC, including service on advisory committees, mentorship of students, financial or in-kind support. etc.
